ELIGIBILITY
Voluntary participation. Enrolled youth and families of the HCN. Child having needs in two of the following areas: · Severe Emotional Disabilities · Academically Challenged · Medically Fragile or Physical Disabilities · Child Welfare Services · Other Interventions not successful over time · Placement In or At-Risk for Out-of-Home Placement ---------------------- Universal application can be applied to individuals of all ages, settings, and situations ----------------------- A service appropriate for any agency or individual wanting to learn about HCN SS -CFS and for those working with HC tribal members. ---------------------- Enrolled members of the Ho-Chunk Nation, other tribal members and youth, families and participants receiving services from Child and Family Services ---------------------- Primary service is to enrolled youth of the Ho-Chunk Nation ages 15-21 who have lived in out-of-home placements for more than six months. Guidelines also apply to other Native American youth living in Ho-Chunk communities. ---------------------- Target population is Native youth in middle or high school students. BRF only. Geared toward those interested in improving their academic standing and need nurturing with confidence, self-esteem (emotionally, spiritually and physically). ---------------------- Target population are enrolled members/parents of the Ho-Chunk Nation children. Service geared toward caregivers seeking advice and counsel from experienced (but by no means perfect) parent. ---------------------- Serves tribal youth, ages 10-17, and families who may be involved in county juvenile justice court systems. |

DESCRIPTION
---------------------------- Initiative offers a child and family-centered team approach to respond to individuals and families with multiple, often serious needs in the least-restrictive setting possible. \ -------------------------------- A Creative Tool for Individuals and Organizations to learn about self and seven teachings in Native American context. Training engages participant creativity and program solving by bringing about a transformation of “self” while deepening the participant’s knowledge and relations with our living earth—it’s abundant life, system and wisdom. ---------------------------------- Universal application can be applied to a variety of settings, individual and group. Ho-Chunk Cultural information, guidance and assistance with presentations, culturally responsive assistance and appropriate materials. ----------------------------------- Learn about intergenerational trauma and ways to work through grief and loss. Healing from unresolved grief and intergenerational trauma, examining the way our belief system shapes our grieving process, identification of cultural traditions and practices that can enhance the healing process and healing unresolved grief in the community. ----------------------------------- Assists youth in gaining life skills to become responsible and productive adults, strengthen their cultural values and identity, and improve their self-esteem. Specific services provided includes: preparation of an Independent Living transition plan; case management and coordination; referrals to community resources; home and money management, food prep and nutrition, health/care hygiene, educational planning & job seeking skills and housing issues. Participation includes monthly visits/contact while enrolled in the program. Educational training vouchers are also available for eligible participants. -------------------------------- The program assists youth with skill-building necessary to compete in the educational realm and through life. Program participation is weekly at BRF Middle and High School and in referrals for tutoring and role models. Geared. ----------------------------------- Individualized program geared toward coaching parents and providing support to address their child’s challenging behaviors. Offers strategies designed to provide healthy and appropriate caregiving. ----------------------------------- CSP strives to advocate for youth to receive services that are culturally appropriate. Through the identification of interests and skills, appropriate placements and services are offered to the youth and their family. |
